What can I expect?

All patients referred to Endodontics & Dental Traumatology by their dentist will undergo a 30-40-minute assessment consultation. These consultations are available Monday through to Thursday and are arranged by contacting our friendly reception staff. Once an assessment consultation booking has been made, you will be sent a letter containing details about your booking and the practice.

​Your assessment consultation will include all examination, radiography and sensibility testing required in order to ascertain a diagnosis and formulate an appropriate treatment plan for you. Should endodontic therapy be required, a treatment plan will be provided to you which outlines the number of visits, what each visit entails and the costs involved.

Medical and Dental History Form

As a new patient to Endodontics & Dental Traumatology we will request that you complete a Medical and Dental History form. This is to provide our Endodontists with a comprehensive overview of your current and past dental and medical health. This information is essential for treatment planning purposes and providing safe treatment of the highest clinical standard. For your convenience, we have made this form available on our website for you to download.

Please email any completed forms to reception@drtaylor.com.au or bring them with you on the day of your appointment. Alternatively, this form can be completed at our practice in just 5-10 minutes prior to your appointment.

All information provided will be treated with complete professional confidentiality.

 

 

Payment Methods

It is our policy that your care is paid for at the time of treatment. For your convenience, you will be able to claim your health fund rebates at your visit through our HICAPS terminal. We accept EFTPOS, Mastercard, Visa, American Express and personal cheque.

Operating Hours

Our practice is normally open for assessment consultations Monday to Thursday, with procedures booked Monday to Thursday. Patients are only seen by appointment.

Appointments

Your appointment time is reserved exclusively for you. We will endeavor to be on time for you and would appreciate your punctuality. Please bring referral letters and any relevant x-rays given to you by your dentist to your consultation appointment with us. Alternatively, your dentist may send these directly to us.

Cancellation of Appointments

If you are unable to keep your appointment, please give us as much notice as possible. Less than 24 hours’ notice may incur a cancellation fee.

Fees and Payments

All fees are due on the day of assessment or treatment. Our practice accepts EFTPOS, Visa, Mastercard, American Express and personal cheque. HICAPS is also available for health fund rebate processing. Additional details of treatment fees specific to your individual requirements will be provided to you after your consultation.

Department of Veterans’ Affairs (DVA)

You must advise reception as soon as possible of your DVA status. Should you arrive at your consultation without prior notice of your DVA status you may be required to pay normal fees for the service.

Upon arrival

In order to provide treatment of the highest standards we need to gain a comprehensive understanding of your current and past health. You will be requested to fill out a Medical History form so please bring with you to your initial consultation your reading glasses and a list of any medications (including vitamins).

Parking

Paid parking (up to two hours) is available directly outside the building. Free parking (up to two hours) is located on Strickland Crescent, adjacent to our clinic. There is also a carpark located on Geils Court that offers paid parking (longstay) and another carpark on the corner of Strickland Crescent and Kent Street (opposite our practice) that offers longstay parking.

Location

Our practice is located on the second level of 2 Geils Court in Deakin. The building is situated on the corner of Strickland Crescent and Geils Court. Refer to the map for further detail.

Please advise us on booking if you have any mobility restrictions that may prevent you from comfortably climbing one flight of stairs.
